Incident Record Forms

Many neighbour disputes can be sorted out by talking calmly with the other person. Worthing Homes will deal promptly if this approach fails or if someone's behaviour is Anti-Social. We can only act against culprits if we have evidence. You can help by keeping records of everything that happens.

The incident record forms you have been issued with are to enable you to record incidents similar to those that you have already made complaints about. It helps the officer dealing with your case to build up a picture of what incidents have taken place and how serious these incidents have been so that the most appropriate course of action can be taken to deal with your complaints. To take legal action we need a carefully written-down description of every incident.

How to complete the incident record forms

The incidents sheets are your own personal record of what you see or hear, you cannot write down something that other people (including your wife, husband or partner) have witnessed. They must complete their own incident record sheets.

You must fill in the diary as soon as possible while the incident is still fresh in your mind. Do it on the same day if you can.

Date - Please enter here the date of the incident.

Time of incident - Please enter the time the incident started and state the duration - length of time the incident lasted for.

What happened? - Write down everything you see and hear in as much detail as possible. Please give names & addresses of those responsible, if you don't know write down "don't know". You will need to include swear words, this is much more effective than "he used abusive language". We are sorry if you find this upsetting, but you will have to do the same if the case is going to stand up in court.

Any witnesses? - please enter the names and addresses of all people who witnessed the incidents and ask them to complete and return to Worthing Homes their own incident record sheet.

How has this affected you? - write down the way the incident made you feel. Include its effect on people who live with you. For example has it disturbed your sleep; frightened your children etc.

Have you reported it? - enter here all agencies that you have reported the incident with, including any agreed follow up action.
